I am thinking that I might need to reinstall Tiger. Do you (or anyone) have thoughts on that, and the best way to backup and then restore my applications and files if I do an "erase and install?"

I'm always hesitant to recommend such a drastic measure because it involves backing up your Mac.

It hasn't helped. And I can't figure out how to restart with the Tiger CD to repair permissions from there; can you advise?

I'm moving next week, so my disk is packed up. Let me see if my memory serves me correctly.
Insert the Tiger CD.
Restart and hold the C button during this which will cause your Mac to use the CD as the start up disc.
You'll see the installation screen come up. I believe you can select DISK UTILITY from one of the menus (file menu?).
Highlight your hard drive. In the right hand portion of the window, you'll see Verify and Repair Permissions. Select Repair Permissions. I usually run this twice. You can also repair your disk which won't hurt.
In order for your Mac to do this correctly, it must be done when using another startup disc. In this case, the Mac OS Tiger CD will do just fine.
Have you uninstalled Missing Sync? That's important if you plan on going back to Palm Desktop.
I'd try unistalling MS, repairing permissions, try getting the sync process working and then enabling the iSync conduit.
If your Treo has all of your calendar dates/contacts etc, then you can set your conduits to overwrite the desktop (if you choose to reinstall Tiger). To be on the safe side, I'd suggest copying the Palm folder (with your User folder) on a CD.
Sorry again for the late response. When I have "tech" problems, a day can seem like an eternity.
