Questionnaire for Treo - Treo Forums

I have had an interesting question come up at work today. We are trying to ask Sales Reps from out company to fill out a 4 question questionnaire in the field. The sheet is simply 4 short questions that are Y/N answers. The reps will be answering these questions several times a day for the next few months. Instead of making them go through the trouble of writing everything down, and mailing/faxing the questionnaire in. I was thinking maybe we could have their Treo devices automate the process. They all have some form out Outlook email capability on their Treo's. Do you know of a way of making a form no their Treo, they could fill out electronically, and then just email in, all from their Treo?

I envision short quetions, followed by 2 boxes, labeled Y and N. Then the user just taps whichever box is the answer, and that box gets a check mark or an x through it, or something. Then they just send the sheet into someones email here. The check box does not even have to be anything more than a picture. There should not be too much data. Someone here could manually read the answers out of the email.

If you know anything about this, or anyone who might, I'd appreciate it.

Couldn't you just make something in an excel spreadsheet?

You can make a draft copy in outlook or word, or excel on your work pc with an area to type Y or N to the question. Then you can copy paste info into an email and then send to the treos. When the treos reply they can type their answer to your questions in the text.

Or if you are not sending multiple times but they are going to respond multiple times have them save email as a draft and copy paste into a new email to send to your office with their answers.

show off... Good idea! I was thinking of something along those lines, but I couldn't quite find the words
