pre-paid cards - int'l cards - how would you do this?
with a prepaid card, you first dial a toll-free number, and then you input your unique access number. I have done that using a "favorites" for the toll-free, and then using the "extra digits" feature for the access number. But what I would like to do then is have another "extra digit" for the actual int'l phone number, say 011-504-843-3457. Is there a way to have two "extra digit" numbers? Is there some easier way. I could just memorize the final int'l phone number, but I am getting too old to memorize stuff like that (unless I have to). Any suggestions? Thanks.

You can place multiple extra numbers by keep inserting pauses between the numbers.
I think you are wanting to use your access code then select contacts and dial that number. I don't know how to do that.

You're off to a good start. If you want to dial the same phone number each time following the calling card, you can do that with Menu -> Edit Favorites Button -> More -> Extra Digits. But if plunker is right and you want to be able to dial a different contact each time, Palm's phone app can't do that. You may want to look into 3rd party solutions like TakePhone. A kludgy solution would be to set up a calling card favorite as above for each f your international contacts.
Good luck.
