Speed Dials Don't Work, Dyslexic DTMF, Problem sending Correct Digits

Problem in brief:
I call ANY voicemail or other IVR system, "quickly" enter 1234 (or any other series of
digits) on my phone, and the system on the other end hears 2134 or 1243, even though I see on my screen 1234. If I re-enter slowly, it's ok. This happens especially with speed dials.
This is not a new problem; had it since I got the 650 in May'05. My boss has had same issue and another colleague just got the 650 in the past month and he has the same issue.
Things I've tried to no avail:
1. If I turn off Long DTMF Tones in Phone Options, that improves my rate of success when dialing quickly or when using speed dials but it still only works 75% of the time.
2. I've tried doing a hard reset, erasing all data, NOT RESYNCING ANYTHING, both PC and Wireless.
3. I've updated to the latest Verizon-Treo software release.
4. I've had a replacement sent (to confirm if a Hardware problem).
What I'd like to know:
Obviously if anyone has a fix, great. But the least I'd like to know from anyone out there (ON ANY CARRIER) is whether they've had the same problem.
I don't do alot of entering numbers on my phone. If you are saying that speed dialing (the one touch button dial on the treo) is causing problems I am not experiencing any with my phone.
You might try entering a pause between numbers on your phone. I think the treo uses a comma for the pause....but I am not sure.
I use Verizon and have had no problems with Speed-dialed phone numbers or "extra digits" I have programmed in, at least not phone the phone (BT is another issue.)
Also, before I saw the "extra digits" option, I entered the numbers manually and had no problems.
Just a thought, but have you considered Takephone? I know it's been mentioned here for folks that are dialing calling card numbers (long and need pauses).
When I dial into voicemail, selecting numbers is rather slow to get right.

Anyone out there still having this problem? It seems to be an issue with older vm systems (we have an old Audix system), and it's driving my CEO nuts. I will try the TAKEphONE software...
Your quality signal will affect your call dialing also. If I am roaming or in a weak signal area and access vm sometimes when you press buttons to delete or save are not heard or ignored by the carrier.

Nope. Pauses don't work. And, I thought she was nuts when she told me she would dial 1234 and it would SAY 1234 on the screen, but the vm system "heard" 1324, or 12234 or something. And really, this is all in the Cleveland metro area, not in any basements or anything, so shouldn't be a signal strength issue. I've now seen a few posts about older vm systems not recognizing the number tones correctly. Grrrr...
