Disappearing Phone functions
The status bar on the Today screen (which includes the Lookup pane and the Bluetooth and Locater icons) disappeared. In addition, the Verizon logo, which appears when the 'phone is turned on or off also disappeared. In addition, the [Send] key doesn't display the call log or redial the last number. Speed dial calls can be made by tapping the on-screen bar, but not by holding down the corresponding key. Finally, when a call is made, I no longer get the page that shows the call progress.
All actual functions work. I can make calls by using the on-screen dial-pad and hitting the [SEND] key. Other functions, including Internet, are normal.
I've tried several soft resets, including removal and replacement of battery. Nothing seems to cure the problem.
Any ideas?

I would have said try Settings > Personal > Today. However, the Verizon bar of info you're referring to isn't there.
How about syncing to back up your data and then performing a "hard reset".

Disappearing 'Phone Functions - Resolution
The answer was, as Chris suggested, a hard reset. I was nervous about the functionality of ActiveSync after a hard reset, but it was really no problem. Before doing the hard reset, I carefully noted down all the settings for ActiveSync (Exchange address, domain, etc.). After the hard reset, I was immediately able to restore my e-mail, contacts, and calendar wirelessly.
